I don't recall the brand, but Costco had it in all the sizes, and also
had pillows of the same memory foam. 

It came packaged in a flat box, about 24" by 48" and 5" thick. I believe
it's the only brand carried by Costco.

The price was around $150.00; my wife paid for it so I can't tell you
the exact amount.

On the weekend we just "installed" the memory foam from Costco. It's
absolutely fantastic. We bought the King size, but it was about 12-14"
too wide. We trimmed it to fit with a sharp knife. There is about 2-3
inches at the foot on the long side of the berth which is not covered,
but that's where my feet are, so no problem. The berth is not
rectangular, but it trims up nicely to fit quite well.
 
Would the queen size be large enough??
 
My wife loves it. 
 
It comes with a nice velour type zippered cover and our fitted sheets
still fit over the foam and the original cushions.

I was thinking of doing the same thing.    The foam only is  2 1/2 in
and the other with the pad is about 5 in. thick.  The COSTCO here did
not have the CA King so it may not be a option for me, so I may just go
with the King.  But you are right seems to be a lot cheaper and easier
than going with the Tuff Topper.  You can also go on line and find the
pads along with all the measurements..

I wonder how hard it would be to get one with the quilted top and trip
it to size.  Sure a lot more economically then the tufted topper.
 
How thick was the foam?

I wasjust looking at them yesteray....   There are two types.  One is
just foam for about $138.00 (King) the other comes with a quilted tuffed
topper for about $158.

That was certainly my concern, but enough on this list had experience
with them to convince me to give it one more try. If you are still
interested, contact Madeline at 312-923-8378, that is her office number.
Apparently they own a 320?. Here are the answers:
 
1--Still have to do the template thing--Jeff Hare reports this is a
pain. 2--Not machine washable as they contain foam etc, but is just like
a pillow top at home where you wouldn't wash any way. Is mildew/mold
resistant.
 
I did end up ordering one and may go the Costco/memory foam route for
the VBerth--
