Jon,
     I bought my boat new in 1994 and have never had a request for any type of survey--I have friends who buy pre-owned boats who have been asked to provide a survey for new insurance, but no one who purchased new and kept the same insurance company have been asked for a survey.    I have Allstate insurance and am very satisfied.
