You can try these guys for shower parts and replacements. I ended up buying
some replacement parts for the stern shower from them:

My head faucet (with the detachable shower wand) is made by Grohe, but I  
cannot see any model ID not can I find it as a current offering in any of  
their website info.

There is a similiar (but not exactly what I have) one for sale on the  
Catalina Direct website...

If I were to do it over, I'd find a single-handle version to make it  
easier to adjust the water temperature.
