David,
There are many possibilities. There is a check valve in  the fuel return line on  our series that should be removed as it tends to let air leak back to the injectors…this one usually only exhibits itself after sitting for several days without running. A second common problem is the nickel crush washer on the top bleed screw just before the injectors. If this is over tightened, it will exhibit the symptoms that you describe. Finally, if any fitting in the fuels system is loose, it will exhibit these symptoms…take a set of metric wrenches and start snugging them up…

On your second question, pick your poison. I fill the tank at every opportunity. First, to keep the overall fuel content as fresh as possible on average and second, to minimize condensation. That said. I don’t hesitate on a long run to carry it on down to the last few gallons. I don’t like to go past that point as I don’t want to have to bleed at an ill timed juncture and I always assume there is a certain amount of crud that  gets concentrated in the last few gallons that I don’t wish to run through my filters, but this mere conjecture on my part.
