 If the washer is copper one can heay it red hot then quench in water to anneal it (soften it) then brush off and oxide without scratching the surface and it should be ready to go.

I had this problem and it was definitely reuse of the washer that was the
problem. It can be reused but only if you do not overtighten.  There was
an article somewhere I read that stated you real should just 'snug it up'.
Any real torque is not a good idea.
